I have an employee who earns commissions on top of regular salary occassionaly. I can get the federal taxes to calculate but I can't get the FICA/Medicare for employee and employer to calculate. Any suggestions?

We really need more information. Do you have the commission setup on the left side of the payroll screen ie: the same side as the salary?
