Need 401K Formula for Tax Table-Catch-up Contributions

Can someone please copy their 401K Formula along with the formula for the catch-up contributions and send it to me. This is the first time EVER that we will have someone who is over age 50, has met the $16,500, and wants to do the $5,500 catch-up contribution. My 401K Formula does not include the calculations to do this.
