Payroll Tax--Need help

Converted from One Write Plus 8.0 to PT Accounting 2004. I manualy set up tax formulas (using samples from sample company) and entered tables. However, when I try to enter a payroll check, I get message: "Couldn't find the FICA EE 04 married calculation." I get same message for "FUTA ER 04," FICA ER 04," "FICA EE 04," "IASIT 04," and "IASUI_ER 04." I just have 2 married employees and have entered "all statuses" for Iowa withholding and "not required" on employee information. Since married does not apply to these taxes, why am I getting these messages? Appreciate your help.

that means those codes are not found. You must have a spelling error.

Thanks. I found a couple of spaces missing and looks like payroll checks withholding will work now except for Iowa State withholding tax. Can't seem to come up with right formula (all filing statuses). I do have table entered. Does anyone have Iowa State formula? Thanks.
